Why “Heart Team” approach in cardiovascular treatment?

Changing the trend of clinical practice, there is an increasing emphasis on multidisciplinary decision-making in various practice guidelines for cardiovascular diseases. When one clinician takes a decision alone there is the potential for referral bias wherein the doctor my overprescribe a certain procedure or avoid doing it based on his own opinion of the procedure.

A  team-based care has great merits. By bridging together specialists of different backgrounds, this approach acts to promote discussion with the principal goal of offering a balanced, complementary, evidence- and experience-based approach to patient care.

Decision between coronary artery bypass grafting (CABG) or percutaneous coronary intervention (PCI) can be made more efficiently if heart team debates the long term benefits of each procedure as compared to short term difficulties in performing it. Both, condition of coronaries and medical co-morbidities have to be taken into account before deciding the most reasonable procedure.

In case of valvular or congenital heart disease (birth defects) a heart team can make a precise decision on timing and nature of surgery best fitting the case.

For these reasons, four major cardiovascular associations—

• the Association for Acute CardioVascular Care (ACVC),
• the European Association of Percutaneous Cardiovascular Interventions (EAPCI),
• the European Association for Cardio-Thoracic Surgery (EACTS), and
• the European Association of Cardiothoracic Anaesthesiology (EACTA)

—have jointly recommended the formation of a heart team to ensure multidisciplinary decision-making which will reduce specialty bias, promote evidence-based care, and help patients make informed decisions.
